IMPORTANT FERTILITY COACHING INFORMATION

Revolve Fertility is excited to offer the fertility coaching services of Dr. Caitlin Fanning, but before you purchase, it is important that you clearly understand the nature of the fertility coaching services. While Dr. Caitlin Fanning is a licensed Naturopathic Doctor in the state of California, Dr. Fanning does not provide any medical advice, diagnosis, or treatment through fertility coaching. Fertility coaching is intended to be strictly educational and supportive in nature. This means that as your fertility coach, Dr. Fanning will not be acting as your doctor. She will not diagnose, prescribe medications, order diagnostic laboratory tests, or otherwise be practicing medicine. Instead, she will be your guide and educator, providing consultation and coaching on your fertility journey from a naturopathic perspective. Ultimately the naturopathic medicine philosophy is based on the principles of putting into place the foundations of health and removing any obstacles so that the body can do its job and function as intended – and the goal of fertility coaching is to help you discover those obstacles and implement and stay on track with those foundations to promote optimal fertility! 

You will need to maintain a doctor/gynecologist licensed in your state who can provide medical care to you. Fertility coaching is designed to be in addition to and is not a substitute for the advice, diagnosis and/or treatment of a licensed healthcare provider in your state. In her role as a fertility coach, Dr. Fanning can consult with you and provide recommendations and suggestions for you to discuss with your medical providers, but Dr. Fanning cannot alter the treatment plans of your medical providers.

Please read these Fertility Coaching Terms of Service carefully before purchasing 1:1 Fertility Coaching with Caitlin Fanning, ND. By purchasing 1:1 Fertility Coaching, you acknowledge and agree to the following:  

• I understand that while Caitlin Fanning is licensed as a Naturopathic Doctor in the State of California, she will not be acting in her capacity as a doctor or as one of my doctors when providing fertility coaching to me. I understand that fertility coaching is not the practice of medicine. 

• I understand that information discussed during fertility coaching is not medical advice and should not be treated as such. The information provided by Caitlin Fanning during fertility coaching is in no way intended to replace the information provided by my doctor(s).

• I understand that the fertility coaching services I receive from Caitlin Fanning do not create a doctor-patient relationship between me and Caitlin Fanning. I understand that I am strongly recommended to maintain medical care with a licensed health care professional in my state while engaging the fertility coaching services of Caitlin Fanning.

• I understand that the purpose of fertility coaching is to complement my medical care and to help me have an empowered relationship with my fertility health. It is Caitlin Fanning’s role to partner with me to provide support and education as I create an action plan for my fertility. I understand that when providing fertility coaching to me, Caitlin Fanning will function exclusively as an educator and consultant and will not be diagnosing or treating any health condition I may have. I understand that any information, recommendations, or suggestions provided by Caitlin Fanning with respect to lifestyle, dietary or herbal supplements, nutrition, stress management, exercise or otherwise, are intended as education and information only, and are not intended as medical advice, diagnosis, or treatment of any disease or condition or a substitute for professional medical advice, diagnosis or treatment that can be provided to me by a licensed healthcare provider.

• I understand that it is my responsibility to discuss all information provided during fertility coaching sessions with my primary health care provider or other licensed health care providers/specialists whose care I may be under. I am especially advised to consult with a licensed healthcare provider prior to changing any medications, taking any dietary or herbal supplements, using any treatment for a health problem, or undertaking any significant changes to my health care regimen. I agree not to disregard professional medical advice or delay seeking professional advice because of information shared during fertility coaching sessions.

1. I understand that Caitlin Fanning will not order lab tests for me, but I may choose to share the results of any lab tests that I have done or any of my health information with Caitlin Fanning. Additionally, Caitlin Fanning may make suggestions for lab tests that I can discuss with my doctor(s) to assess if such lab tests would be applicable and/or useful. 
2. I understand that discussions or reviews by Caitlin Fanning of any of my lab tests and/or functional diagnostic tests are solely for educational purposes and to enhance my knowledge and assist me in the development of a lifestyle plan or plan to work with my doctors, and not to diagnose any health condition.
3. I understand that if Caitlin Fanning suspects the existence of a disease or medical condition requiring diagnosis, I will be informed of this suspicion and recommended to consult a licensed medical professional in my state for diagnosis and treatment.

• I understand that because Caitlin Fanning is not acting in the capacity of a licensed medical provider and cannot diagnose or treat any health condition, Caitlin Fanning will not be held liable for failure to diagnose, identify, treat, or prevent any health condition I have.

• I understand that no results of my participation in Fertility Coaching are guaranteed. 

1. I hereby release and agree to hold harmless Caitlin Fanning from and against any and all liabilities, losses, damages, costs, expenses, and any other claims relating to or arising from the fertility coaching services I receive from Caitlin Fanning.

I have read the above information which explains the role of Caitlin Fanning and fertility coaching. By purchasing 1:1 Fertility Coaching from Revolve Fertility I understand, accept and agree to the Fertility Coaching Terms of Service.
